Job Description


Senior Backend Developer (Fully Remote, Node.js)

Contract: Outside IR35 Duration: 3 months, potential extension

Start: ASAP

Day Rate: £550/day

Location: Fully Remote (UK hours)


The Role

You'll join as a peer alongside our existing senior developers. You'll own features and

integration workstreams end to end, with collaboration through code review and shared

technical decision-making.


The work splits roughly 75/25 between API development for our FHIR-based clinical platform

and third-party integrations. Genuinely interesting domain problems, not generic CRUD.

Features are scoped collaboratively with product. You'll own implementation through to

deployment, with your peer as code review partner.


What you'll work on

  1. Building and extending RESTful APIs in TypeScript and Node.js
  2. Integrating with third-party platforms including cal.com, Awell (clinical workflow engine) and healthcare data systems
  3. Working with FHIR R4 resources and clinical data models within Medplum
  4. Contributing to our platform migration and new feature development for H1 client launches
  5. Participating in technical decisions and architecture discussions


What we're looking for

Essential

  1. Strong TypeScript and Node.js experience in production environments
  2. Solid API design skills. You've built and maintained systems others depend on.
  3. Experience integrating with third-party APIs, including handling auth, webhooks and the inevitable quirks
  4. Strong testing practices. You write tests because you've been burned when you didn't.
  5. Comfortable working autonomously once you understand the context
  6. Clear written communication. We're a fully remote team.


Strong Preference

  1. Experience in regulated industries (healthcare, finance or similar). You understand why compliance matters and won't cut corners.
  2. Familiarity with FHIR R4 or other healthcare data standards
  3. GCP/Firebase experience - this is where most of our infrastructure is hosted


Nice to Have

  1. Direct experience with Medplum or similar FHIR platforms
  2. Startup or scale-up background. You're comfortable with ambiguity and moving quickly.


Practicalities

  1. Fully remote, but we need reliable overlap with UK working hours for collaboration
  2. 3-month initial engagement with likely extension. Backend capacity is our constraint for multiple client launches.
  3. Outside IR35. You'll need your own limited company or umbrella arrangement.
  4. We'll onboard you properly on our clinical safety and compliance context

About Perci Health

We're a UK healthtech startup building a regulated digital platform that connects people

affected by cancer with allied health professionals, including dietitians, physiotherapists and

psychologists. We're scaling rapidly with major enterprise client launches in H1 2026 and

need to expand our backend engineering capacity.
