Job Description


We are looking for a passionate and skilled Band 6 Caseload Manager/Lead Nurse to join our Thanet Community Nursing Teams, to be based at our brand new £10million state-of-the-art Thanet Health Hub.

Our new hub directly supports the Governments new 10-year plan, that places community care at the heart of our health system.

This plan represents one of the most significant shifts in NHS history: moving care from hospitals into neighbourhoods, from reactive treatment to proactive prevention. And here at Kent Community Health, we're right at the forefront.

Opened in Autumn 2025, our new Thanet Health Hub consists of an NHS community diagnostic centre, a range of community NHS services, including community nursing, relocation of an existing GP surgery and signposting and support to access health and care services provided by voluntary sector organisations, such as Age UK.

You will play a vital role in our multidisciplinary services; managing patients with complex needs in the community. You’ll have the freedom to plan care in your own way, with time to develop your assessment and treatment skills, with access to great supervision and a clinical lead to make sure you are on the right track.

You will be supporting junior colleagues with clinical development, helping them to reach their potential and develop the workforce of the future. If you are applying for a Community nursing role, it is essential that you hold the Specialist Practitioner (District Nursing) qualification but we will consider development and secondment opportunities.

Additional Working Pattern: Hours between 08:00 - 18:00 Mon-Sun
